Free Google Forms QR Code Generator

Create a QR code that opens your Google Form directly on the respondent's phone. Perfect for classroom quizzes, event feedback, employee pulse surveys, academic research sign-ups, and any form you want people to actually fill out.

Paste your Google Form link below. The short forms.gle/... version (found via the Send button in Forms) produces the cleanest QR.

Short forms.gle links are preferred. Full docs.google.com/forms/... URLs also work.

Boosting Response Rates with QR Codes on Forms

Response rates on Google Forms live or die by friction. A link in an email gets ignored. A URL on a slide gets mistyped. A QR code projected on a screen or printed on a handout eliminates the typing entirely, and response rates typically double or triple.

Teachers put QRs on the whiteboard for exit tickets and pop quizzes. Event organizers print them on badges for session feedback. HR teams use them for anonymous culture surveys posted in break rooms. Researchers put them on recruitment flyers for study sign-ups.

The pre-filled link trick is especially powerful for multi-location organizations: one QR per store with the store code pre-tagged lets you see per-location results in the same spreadsheet without respondents having to think about it.

Frequently Asked Questions

Should I use the short forms.gle link or the full URL?

Use the short forms.gle/... link whenever possible — it produces a smaller, denser QR that's easier to scan and prints crisper at small sizes. The full docs.google.com/forms/d/e/.../viewform URL works too, but creates a busier QR.

Does it work if my form requires Google sign-in?

Yes, but respondents who aren't signed in will be prompted to sign in before they can submit. For public-facing forms (events, open surveys), uncheck "Limit to users in your organization" and "Collect email addresses" so anyone can respond. For internal use (schools, companies), the sign-in requirement is a feature, not a problem.

Can respondents fill it out on mobile?

Yes. Google Forms is fully responsive — multiple-choice, text, date pickers, and file upload all work on phones. Scanning the QR opens the form in the mobile browser, respondents fill it in, and tap submit. No app required.

Can I pre-fill answers via the URL?

Yes. In Google Forms, open the three-dot menu and select "Get pre-filled link." Fill in the answers you want pre-populated, then generate the pre-filled URL. Use that URL in the QR — useful for tagging responses by location (one QR per event, with location pre-filled).

How do I track which QR drove which response?

Add a hidden "source" question to your form, then use a pre-filled link (see above) that sets the source to a unique value per QR. Each location, event, or printed material gets its own QR with its own source tag — the responses segment themselves in your spreadsheet.
